From what I can see it's made by Conexant, they made Video encoders on early versin Xbox's. The little white label should have a model number you could Google.

It's a modem, because it has dual RJ-11 jacks and a transformer and bridge recitifier. All because phone lines are AC.

Some modems have audio jacks in the back so you could plug in a microphone headset and talk to people over the phone line without bothering with a phone.

Has anyone even tried searching for it? :?

It's a PCI combination modem and sound card. Completely useless nowadays since nearly all PCs come with audio hardware and hardly anyone uses modems anymore.

Yes it is. I have a near exact card like that down in my basement.

Bicostp is also correct that it's an outdated card. Throw it out, recycle it, whatever. There is no modern or semi-modern computer that can accept such a card. And no there is no way to mod or make an adapter for it, again it's useless throw it out.

Not the way I would put it but yes that would be a good way to saying why it's not working.

I think it's PCI. Aren't the end connectors on ISA boards fatter and farther apart? :?

Thats PCI not ISA. Anyways an ISA card would have loads more dust and grime on it then a PCI card  Looks like it has a Gameport on it as well.

I had a very similar one by them in my HP Pavilion 4450. It is a PCI multi-card with a built in soundcard, modem, and joystick port. Damn thing gave me so many issues cause my PC overloaded thinking there were 2 pci slots being used up by it, not 1. The card is a pain in the ass. If it died, I salute you it's an excuse to get a new sound card (and a new dial-up modem, if you still use it).
