Some perf board as a guide and a steady hand with a drill makes for excellent speaker holes.
If you make cases by hand like I do, I recommend getting an IRWIN Unibit - it drills holes of just about any size in mere seconds. I got a Home Depot gift card for Christmas (WOO!) and so was finally able to afford one - they cost like thirty bucks!
I just won a Flashback 2 for a decent price on eBay so more progress soon. This unit will play both cartridges and the built-in games the Flashback comes with.
I'm going for a real clunky retro look - am I succeeding?

I have several of those step bits, they are amazing. If you have a drill press and a steady hand you can use the next size up to chamfer the edge of the hole you just drilled.

If you look closely at the button on the right you'll notice that I drilled a bit too far and did just that - unfortunately that made the button sink down a bit into the case which is not what I wanted. Grrr... So, I got this really good deal on a flashback with one little issue - the stupid thing won't register the "up" direction. I thought that it might just be a loose connection or something so I opened it up, but I just can't get it to register. I'll fiddle around with it some more but I suspect it's not going to be anything that I can fix. Man...

No, I have tested it just about every way I van think of and I am still having issues. After all, it is just a cheap OAC with a glop top. Anyone have a Flashback 2 they would be willing to sell me?

How does that step bit do on plastic? I've really only ever used them on very thin metal, and I'm just wondering if they're any less prone to cracking plastic than my current method.

Tibia if you are drilling through brittle plastic (Like Atari cases) you can't go wrong with some cheap Forstner bits. Once I moved to these I have never cracked an Atari case.

Do you need a piece of wood or something behind the plastic when you drill (Like you have to do with a spade drill bit) ?
